Admissions

Basic eligibility

Review the current entry requirements and required documentation before contacting CRC to complete the phone assessment.

  1. Applicant must be at least 21 years old.
  2. Applicant must have completed a 28-day residential treatment program. CRC can provide door-to-door transportation by picking up the applicant directly from the treatment program.
  3. CRC does not accept applicants with sex-offender status.
  4. Provide a printed list of all current medications.
  5. Provide a copy of a current photo ID.
  6. Provide a copy of the applicant's Social Security card.
  7. Provide a chemical-dependency assessment from the inpatient treatment program.
  8. Provide a psychiatric assessment, if applicable.
  9. Provide the discharge summary at the time of discharge.
  10. Provide a physician's letter confirming that the applicant can climb stairs and complete a daily household chore, such as sweeping or mopping. CRC is located in an old school building with many stairs, and residents use them throughout the day.
  11. Provide a current tuberculosis test completed within the last six months.
  12. Fax all required information to (217) 533-4303. Any CRC staff member can retrieve the documentation.

Call CRC after all documents have been received.

The prospective resident should call (217) 533-4441 between 8:00 a.m. and 3:00 p.m., Monday through Friday, to complete a phone assessment. If accepted into the CRC program, the applicant will be placed on a waiting list. Wait times vary based on bed availability.

Applicants are encouraged to call at least weekly to ask about available bed space. Please contact the CRC office with any additional questions about the program or admission process.
